Description: This education module on management of medication history for patients in Emergency department is a patient safety initiative for patients seen in ED. The session is based on studies done in the recent years on patient safety which indicate that errors on medication management following additions, or changes in ED can have adverse effects on patients. Thus it is important that a best possible medication history (BPMH)is created while the patient is in ED and adequate education on new medications or any medication changes is done before discharge from ED.
