Managing Medication History in Emergency Department

At the end of the session, learners will;

(1). understand what Best Possible Medication History (BPMH) is including in ED.

(2). be able to identify and verbalize the benefits of completing a BPMH

(3). express the desire complete BPMH on ED patients even more with patients with CTAS score 3 and higher (1 and 2).

(4). verbalize the importance of medication management teaching to patients on discharge from the hospital

(5). express desire and willingness to provide patient(s) with Medication Information booklet to patients on discharge from ED
