Formative Evaluation:  Done during the class to test for understanding.  The students are given a problem to work on individually or in groups of two.  They should use their calculators to help them solve the problems.  The solutions will be reviewed in class.  Give the students 5 minutes to work on the problems and review the results and questions as a class.

The teacher will also monitor class participation in discussions, and responses to questions.

Summative Evaluation:  The teacher will ask the students to return the Quadratic Formula Worksheet and evaluate responses based on the following rubric:

Not yet accomplished

Answers both questions incorrectly and showed a limited number of the required elements in the solution.

Basic

Correctly answered at least one of the two problems.  Some errors in applying steps;  may have forgotten about the +/- or 2a.

Satisfactory / Good

Correctly answered at least one of the two problems.  Applied each of the steps correctly.

Above Average

Answers both questions, using all the elements required and showed all of their work to arrive at the correct answers.
