Lifestyle Disease: So what?

As the Wellness Director, you will decide which lifestyle disease is most prevalent at your site...Choose from the 3 below. 

First, decide which disease you will be responsible for: 

Heart Disease
Type II Diabetes
Cancer

Next, decide your final product:  (More information under the EVALUATION section of this webquest)

I Am Poem (the "I" is the disease) or ABC POEM
Journal /Diary of a person living with_________ (for example, My Life with _______)
Cartoon
Fable (Once upon a time....pick characters or animals)  Remember a fable is a lesson learned!

Information that needs to be included in each final product:

Prevention:  what to do to prevent the illness from occurring
Risk Factors:  what puts people at risk for getting the disease?
Warning Signs:  how do they know they are developing the disease?
Treatments/cost:  what now...when they have it?
What is life like living with the disease?

You have the freedom to choose the disease and the product you want to use at your site.  Remember it is aimed at students around the age of 12 so keep it simple, but informative and accurate.
