  • Purpose of the Study: The purpose of this survey is to collect information about the use of zWebQuest to develop curriculum material for classroom use.
  • Benefits of this Study: The results of this survey will help survey creators understand how and where the website is being used, what additional features users are interested in having on the website, and how educators are using or planning to use the tools of this website in their classroom. The results will be submitted for professional presentations and publications.
  • Risks or Discomforts: No risks or discomforts are anticipated from taking part in this study. If you feel uncomfortable with a question, you can skip that question or withdraw from the study altogether. If you decide to quit at any time before you have finished the questionnaire, your answers will NOT be recorded.
  • Confidentiality: Your responses will be kept completely confidential. The survey will not ask any confidential information. We will NOT record your IP address when you respond to this survey. The research records obtained from this study will be kept as confidential as possible. They will be kept for a minimum of 3 years and only certain people may need to see these study records: the Principal and Co-Principal Investigator, the USF IRB office and the staff, and the Department of Health and Human Services. Since this is an anonymous survey, no one will know your name; therefore, if we publish the results from this study you will not be identified.
  • Decision to quit at any time: Your participation is voluntary; you are free to withdraw your participation from this study at any time. If you do not want to continue, you can simply leave this website.

A note on privacy
This survey is anonymous.
The record kept of your survey responses does not contain any identifying information about you unless a specific question in the survey has asked for this. If you have responded to a survey that used an identifying token to allow you to access the survey, you can rest assured that the identifying token is not kept with your responses. It is managed in a separate database, and will only be updated to indicate that you have (or haven't) completed this survey. There is no way of matching identification tokens with survey responses in this survey.
